Certificate of Registration of Death
  • The Certificate of Registration of Death (Form 14 or the Green Form) is issued by the Registrar of Births, Deaths and Marriages free of charge. It needs to be given to the burial or cremation authority by either the funeral director or the family/relatives.

  • If the Procurator Fiscal has investigated the death, and cremation is required, this form will be replaced with a Procurator Fiscal Certificate for Cremation (Form E1). The Procurator Fiscal will send this directly to the Funeral director.

  • Occasionally a Registrar of Births, Deaths and Marriages may be able to issue a certificate for burial only (but never cremation) where no one has yet been able to register the death.

  • Form 14 and Form E1 are retained by the local authority.

Registration or Notification of Death Form

  • The Registration or Notification of Death Form (Form BD8) is issued by the Registrar of Births, Deaths and Marriages for use in obtaining or adjusting Social Security Benefits or for National Insurance purposes.

  • The form needs to be sent to the Department of Work & Pensions in order that they can deal with the deceased's pension and other benefits.

Extract of an Entry in the Register of Deaths

  • Commonly known as "Copy of the Death Certificate", this is issued by the Registrar of Births, Deaths and Marriages at the time of registration of death.

  • The Registrar of Births, Deaths and Marriages will advise of any further certificate copies required for obtaining Grant of Probate or Letters of Administration to show:
    • banks
    • social security 
    • building societies
    • insurance companies

  • Payment is required to obtain further certificate copies. The Registrar of Births, Deaths and Marriages will advise of the cost involved. Copies of certificates can also be obtained from http://www.gov-certificates.co.uk/.

  • This is the only official document which will be left with the family/relatives after the funeral has taken place.
